The Clinical Program Coordinator will work collaboratively with OptumCare Arizona’s care delivery team and external partners to meet and/or exceed strategic business, clinical, and operational goals. The ideal candidate will have exceptional communication, teamwork, and project management skills. This position reports to the Associate Director, Medical Management.
This role is a telecommuting role and will be based out of the employee’s home. We require Arizona residence, with local travel up to 10% of the time.
Primary Responsibilities:
- Develop and maintain relationships with vendor partners
- Review vendor performance on a regular basis to identify areas of risk and quality improvement opportunities
- Works collaboratively with patients, network providers (PCP’s, specialists, and vendors) and internal staff to assist in managing care, cost, and outcomes across the continuum of care
- Leads and actively participates in interdisciplinary team (IDT) meetings with external vendors and facilitates the coordination of patient care
- Serves as a primary liaison and clinical resource for Medical Management team members
- Review and audit contracted vendors
- Manages projects and interventions from inception to completion

Required Qualifications:
- Current, unrestricted RN or MSW license in the state of Arizona
- 3+ years of experience as a Registered Nurse (RN) or Master Level Social Worker (MSW)
- Ability to prioritize, plan, and handle multiple tasks/demands simultaneously
- Proficient in Microsoft Applications to include Outlook, Word, Excel, and PowerPoint
- Ability to read, analyze and interpret information in medical records to perform quality audits
- Telecommuters must have a dedicated work area that provides information privacy
- Access to reliable transportation that will enable you to travel to vendor sites

Preferred Qualifications:
- Managed Care experience including Medicare Advantage, Commercial and Medicaid products
- 1+ year home health experience
- Experience with planning and leading projects to implementation
- Knowledge of utilization management, care coordination, quality improvement, and discharge planning
- Excellent conflict resolution and verbal, written and interpersonal communication skills
- Independently utilize critical thinking skills, judgement, and decision-making skills
