Your day.
As a Pharmacist you need to know how to:
- Demonstrate competency in reviewing, verifying Physician's orders to ensure that correct dose, frequency, and route have been prescribed. At the time of order entry/verification, using patient profile, screen for medication duplications, drug interactions and allergies.
- Demonstrate competency in reviewing and interpreting and verifying Physicians orders for all LVP, IVPB, TPN, PPN, selected IV pushes, and Chemotherapy. Use patient profiles to assure correct dose, frequency, and route. Screening for medication duplications, drug interactions, allergies, and incompatibilities. Use aseptic technique for all intravenous admixing.
- Demonstrate a basic understanding of the pharmacology and pathophysiology involved which relate to the patient's disease state. Perform clinical duties within the parameters defined by the Medical Staff approved protocols.
- Demonstrate competency in reviewing patient profiles to assure correct dose, frequency, route of administration, allergies, interactions and duplications.
- Understand and perform all technician duties when called upon to fill a technician shift.
- Respond to "Code Blue" and "Trauma Codes" immediately and establish recording timeline of event.
- Be responsible for control and replacement of all drug products in facility.
- Demonstrate competency in providing guidance and insight to Pharmacy Technicians and Students (Interns and Externs).
- Act as educational resource for patients and all health care providers.
- Communicate with other departments concerning drugs and drug related problems.
- Provide specific drug information to patients to aid the patient in proper use of medications outside the health care facility (i.e. Cardiac Rehab, Coumadin Education, and Diabetic Education).
- Perform Pharmacy Tech duties when Pharmacy Tech is not available.
- Participate in department projects, QA and DUR's and complete duties in a timely fashion.
- Be responsible for specific projects or areas of responsibility which change and evolve. These areas of responsibility or projects include; CQI, P&T Committee, TCU, Computer Updates, DUE, Adr, Outpatient Medicaid, Narcotics QA, Non-Formulary drug requests, scheduling, Omnicell, maintain Formulary, and other committees as required.
