Our Bruner Family Medicine Center Clinic is a busy, full-service family medicine clinic with a diverse patient population. We are a NCQA Level 3 Patient Centered Medical Home focused on providing care for all individuals regardless of ability to pay for services. We are committed to caring for the underserved and many of our patients have limited access to care. Currently 30% of our patients have no insurance, 45% have Medicaid, 16% are commercially insured, and 14% have Medicare. We have a large number of bilingual employees and translation services are also available to help us care for our patients.

As a Care Manager you need to know how to:
Collaborate with the health care team to develop a plan of care by participating in patient care
Demonstrate knowledge of the principles of growth and development over the life span and possesses the ability to differentiate the care that is required based on the patientsâ™ current place on the life growth and development flow
Facilitates the safe care of patients including transfer of care to alternate level of care settings, assuring appropriate exchange of information among all caregivers
Identify patients requiring care management and takes the lead as care manager for those requiring intervention, especially with socially complex cases
Participate in professional development activities and internal/external customer service including motivational interviewing
Utilize motivational interviewing skills to support health behavior change in Bruner Family Medicine patients
Provide education to other departments/clinics regarding care management services as requested
Participates in co-visits with providers using a multidisciplinary approach as needed
Serve as support to Bruner Family Medicine for population management, patient self-management, along with support and tracking and intervening with high utilizers
Coordinate patient centered care to reduce fragmentation of services
Interview patients and families to assess current strategies for self-care and available support systems to manage their health
Serve as a support to Bruner Family Medicine in the area of social services, Medicaid applications, disability eligibility, ETOH resources and homeless resources
Use assessment skills to personalize a safe plan of care that is in alignment with providing choice, assuring safety, and is timely and is cost effective
UsesSW knowledge and skills to assure the health care team understands and supports patient self-determination when appropriate
Partner with community resources and community referrals sources as well as build new relationship with the direct community for patient care needs

Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
Knowledge and experience with managed care
1+ year Care Manager experience in Primary Care setting
MSW with Medical /Acute Care experience
Graduate of an accredited school of social work, Sociology or related field.
Working knowledge of Microsoft Office programs
Experience with PC software applications in word processing, Epic, Word, Excel
