Lead Social Worker, Integrated Health - 2100592 Position Summary At Stony Brook Medicine, Social workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement. The Lead Social worker in this area would have a case load and be involved in education, training, and coordination with staff on both the treatment teams as well as the Care Management Department. Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care, and psychiatric patient care. Demonstrates strong writing, follow up, and computer skills. Demonstrates an ability to present material in meetings both on line and in person. Demonstrates ability to lead staff as well as manage a case load. Demonstrates understanding of Medical Social Work as well as Behavioral Health issues including but not limited to: - Psychiatric and Post-Acute Medical resources in community,
- Legal guidelines for Psychiatric plan of care and discharge planning,
- Substance Use Disorder identification, treatment and referral,
- OMH and OPWDD processes and regulations
- Primary Medical and Behavioral Health Diagnosis as it pertains to patient experience, Psycho-Social Evaluation, and Discharge Planning.
- Transition to care including Psychiatric, Substance Use Disorder, SNF, and SAR placement
- Home Care and Assisted Living
- Geriatric Medicine, and Behavioral Health issues
- Adolescent Medicine and Behavioral Health issues
- Complex Discharges
- Long Term Psychiatric and Forensic Placements
- CPS and APS referrals
- AOT criteria, SPA criteria
- Veteran Placements
Duties of a Lead Social Worker may include the following but are not limited to: - Chart review and oversight of documentation compliance
- Team discussion/rounds
- Discharge planning oversight
- Patient/Family Assessments
- -Family Meetings
- Outside agency liaison
- Insurance and hospital contacts regarding admissions
- Process improvement and quality improvement
- Project Management for psych/ psych social work initiatives
- -Forms and Charting improvements relevant to topic
- Applications to Acute Behavioral Health: housing, outpatient programs, Case Management
- Discharges to post-acute care
- Guidance and Education to staff on implementation of complex discharges
Selected candidate is on-call, re-call eligible. Qualifications Required Qualifications: - MSW from an accredited Social Work Education School.
- NYS LMSW or LCSW.
- 5+ years' experience post Master's Social Work experience.
- Experience in both Behavioral Health Social Work and Medical Social Work required.
- Demonstrates understanding and knowledge of generational and developmental psychosocial needs
- Ability to work and coordinate with established teams.
- Demonstrates understanding of discharge planning and Long Length of stay barriers and systems.
- Demonstrates understanding of Psychiatric and Medical specific community resources.
- Demonstrates good assessment and psycho-social skills.
- Working knowledge of Long Island and Metropolitan NY community resources.
- Demonstrates patient first, patient advocacy and service excellence.
- Computer literate with strong writing skills.
- Ability to educate and utilize power point and other educational platforms.
- Demonstrates ability for project management and action planning.
- Demonstrates understanding of data collection and analysis as it pertains to quantifying and supporting the work.
Preferred Qualifications: - Bilingual
- Five years or more in Medical and Psychiatric In-Patient settings.
