As a Social Worker you need to know how to:
Provide continuing care assessments, reassessments of patients and families to assist in development of their treatment and discharge plan; identify psychosocial / financial needs; access appropriate services and resources.
Coordinate discharge / treatment plan with medical, interdisciplinary staff to effect timely discharge.
Provide community resources for psychosocial / financial / educational needs of patient /family. Collaborate with agencies.
Provide patient / family education, community education, i.e. advanced directives, domestic violence, child and elder abuse.
Communicate / collaborate with medical, interdisciplinary team staff, patient / family, insurance companies, social agencies to provide quality discharge plan to appropriate, safe, level of care.

Masterâ™s Degree in Social Work (MSW) or equivalent with experience in discharge planning and counseling, required
Minimum of two (2) years of related experience, required
