Responsibilities The Population Health (Healthy Planet) Applications Analyst is the primary analyst supporting the Office of Population Health & Accountable Care Programs through provisioning of focused healthy planet tools/workflows in the following key areas to achieve value-based care: 1) high-risk/high cost populations (e.g. CKD, COPD, HF, etc.) 2) Transitions of Care and 3) Ambulatory Operations. Works closely with others on the application team to coordinate all system issues that arise during the use of the application(s) by the Office of Population Health stakeholders. Knowledgeable about the procedures, workflows, and business operations within the end users organizations. The analyst has in-depth knowledge of the EpicCare Ambulatory system with a focus in population health management and administration and serves as a bridge between the end users, vendors, and other IS support teams. Position will support the Office of Population Health & Accountable Care leadership team and the Healthy Planet platform. The focus of this position will be on implementing Epic healthy planet tools that focus on improving patient outcomes, lowering cost of care, improving care transitions, and improving patient satisfaction scores at the organization for specific patient populations (i.e. at-risk arrangements with payors). This position is located in Sacramento, California. Telecommuting options may be considered based on the needs of the dept/unit and management approval.
This role involves complex projects and demands independent decision making, project management skills and superior customer service interactions with a wide range of constituents. The analyst will partner with UC Davis Health System faculty and staff to develop and implement Epic Healthy Planet (population health) workflows. Responsibilities will include the collaborative design of population health tools, epic dashboards, reporting workbench reports, clinical documentation requirements in the Epic electronic medical record environment; manage design, deployment, testing, and on-going support of Epic systems used in the delivery of population health tools. Additional responsibilities will include data quality validation of the population health Epic dashboards that may include quality scores, process metrics, utilization metrics, cost metrics, and other metrics to be defined.
The analyst will be a leader on the team by partnering daily to monitor responsibilities, tasks, and other duties assigned to the team. Analyst will be available to answer any support/project questions, assist in troubleshooting, give guidance on projects and general next steps, as well as provide detailed status to the team manager. The analyst will help oversee the team's daily responsibilities to ensure tasks are completed on time, and will escalate to manager as necessary.
Incumbent provides advanced level software support to UC Davis Health System staff and faculty in the assigned department and clinics, implementing and supporting specialized, complex distributed clinical information systems and multiple complex ancillary/patient care systems. Aligns assigned departmental technical solutions with UCDHS IT Division goals, guidelines and directives. Incumbent has the responsibility for supporting large, complex and/or unique projects that affect multiple departments and will include product analysis and assessment, recommendation, implementation, testing and roll-out of new technologies in a cross matrixed environment.
For EMR operational projects, this position performs analysis, build and testing for assigned projects. This may include system changes across multiple environments. Coordinate communication, testing, and updating any changes to support documentation for assigned operational EMR changes.
Bachelor's degree in Healthcare Administration, Public Health, Mathematics, Healthcare Informatics or Computer Science or equilvant experience.
Epic Ambulatory ceritification or Healthy Planet certification is required.
Experience with Epic Electronic Medical Record (EMR) build and configuration.
Experience working with multiple clinical information systems or system implementation teams for complex projects and/or application development.
Demonstrated comprehensive knowledge of clinical practices and principles.
Experience with methods and tools for systems development life cycle as related to clinical informatics.
Demonstrated ability to gather information from multiple sources and analyze applicability and appropriateness, and develop an efficient workflow and subsequent computerized process.
Experience with designing and monitoring technology-enabled organizational change.
Experience with quality improvement principles, practices, and tools into system design.
Experience with coordinating system development and implementation with relevant clinical operations initiatives and informatics projects.
Experience with advanced application of adult learning principles and clinical system design standards.
Experience with organizational strategies, business operations, and end-user requirements to develop system design and training projects.
Excellent interpersonal, communication, leadership and change management skills to establish and maintain positive and productive working relationships.
Proficient in office automation software (word processing, spreadsheets, visual presentation, etc.) and strong presentation skills.
Experience working in an academic institution or large integrated health system.
Experience with project management, information systems trends, and professional practice standards.
Epic Healthy Planet, Reporing Workbench, and/or Radar certifications is preferred.
Experience with population health management programs and/or accountable care organizations (ACO) is preferred.
Successfully complete training or certifications as required by hiring manager. Some travel may be required for training or certification
Travel in the Northern California region may be required. Reliable transportation necessary for off-site visits. Mileage and travel expenses will be reimbursed per UC Policy.
Occasional out of state travel may be required to attend training.
