You bring your body, mind, heart and spirit to your work as a Home Hospice Social Worker.
You know youâ™re a guest in someoneâ™s home and you tread carefully but confidently.
You know how to make small talk and answer big questions. You know how to soothe. How to hug. How to travel, and laugh, easily.
Youâ™re great at what you do, but you want to be part of something even greater. Because you believe that while individuals can be strong, the right team is invincible.
Lutheran Hospice Center - Home Hospice is part of SCL Health, a faith-based nonprofit healthcare organization that focuses on person-centered care. Lutheran Hospice has supported terminally ill patients and their families with end-of-life care for more than 30 years and is one of the few hospices in Colorado to be continually accredited by The Joint Commission. With clientele throughout the greater Denver metro area, we proudly provide comfort, dignity and support for patients as they spend the final stage of life with loved ones in familiar surroundings.
Benefits are one of the ways we encourage health for you and your family. Our generous package includes medical, dental and vision coverage. But health is more than a well-working body: it encompasses body, mind and social well-being. To that end, weâ™ve launched a Healthy Living program to address your holistic health. Healthy Living includes financial incentives, digital tools, tobacco cessation, classes, counseling and paid time off. We also offer financial wellness tools and retirement planning.
Together weâ™ll align mission and careers, values and workplace. Weâ™ll encourage joy and take pride in our integrity.
Weâ™ll laugh at each otherâ™s jokes (even the bad ones). Weâ™ll hello and high five. Weâ™ll celebrate milestones and acknowledge the value of spirituality in healing.
Weâ™re proud of what we know, which includes how much there is to learn.
As a Home Hospice Social Worker you need to know how to:
Assess for appropriateness of level of care setting from admission through discharge.
Identify patients requiring case management and take the lead as care manager for those requiring intervention, especially with socially complex cases.
Collaborate with the health care team to develop a plan of care by participating in patient care rounds.
Assure the plan of care is individualized for patients with a chronic disease to support patient/family self-management.
Serve as a support to nursing partners in the area of social services, Medicaid applications, disability eligibility, ETOH resources and homeless resources.
We hire people, not resumes. But we also expect excellence, which is why we require:
MSW, Graduate of an accredited school of social work, sociology, or related field, required.
Current license in good standing as a Medical Social Worker (LSW or LCSW) in the state of practice, required.
Ability to maintain reliable transportation, maintain a current driverâ™s license in good standing and current auto insurance, required.

Now that you know more about being a Home Hospice Social Worker on our team we hope youâ™ll join us. At SCL Health youâ™ll reaffirm every day how much you love this work, and why you were called to it in the first place.
