The Transplant Clinical Pharmacy Specialist will provide clinical pharmacy services to the renal transplant services to the renal transplant service. The Specialist will work as part of the multidisciplinary transplant team in order to deliver safe, appropriate, comprehensive and cost-effective drug therapy for pre-transplant and post-transplant patients. The individual will serve on committees/task forces and participate in projects related to transplant medication use such as drug utilization evaluations and performance improvement initiatives. Responsibilities also include providing educational programs for physicians, nurses, nephrologists, physician assistants, pharmacists, patients, and pharmacy students and residents. Participate in activities during pre-, peri-, and post-transplant phases of patient care. Follow all post-transplant patients admitted to the inpatient renal transplant service. Participate in projects related to transplant pharmacotherapy use such as drug utilization reviews. Precept and monitor Pharmacy residents and students during the Transplant rotation this title also provides clinical evaluations and acts as a consultant to the P&T Committee. The Pharmacy Department at SUNY Downstate is seeking a full time Senior Pharmacist/ Transplant Clinical Pharmacist.
