As an RN Clinical Nurse Care Manager you need to know how to:
Lead care management and care coordination of patients served by the dental clinic.
Be responsible for conducting procedures, assessments, administration of medication, and referral to primary care provider, emergency department, urgent care, and other duties within the scope of nursing practice.
Collaborate with dentist and other members of the health care team on the patientâ™s behalf.
Monitor efficiency and availability of medical services provided at the dental clinic and evaluate outcomes through variance tracking, data collection.
Coordinate the care and services for patients identified as needing assistance, including primary care services and social needs.

Graduation from an accredited nursing school
Active, Un-restricted Registered Nurse License in the state of Kansas
Current BLS certification endorsed by the American Heart Association
Minimum of three (3) years of experience as a Registered Nurse
Bachelorâ™s degree or higher
Current CCM (Clinical Care Manager) certificate
Experience working in a safety net clinic or public health agency
Experience working with special needs populations
At least two (2) years of experience with care management
Bi-lingual and/or multi-lingual
