OBJECTIVE/OVERVIEW: CUHCC’s mission is transforming care and education to advance health equity. The clinic is known for providing high quality care to diverse patients. This position contributes to the efficiency and effectiveness of the clinic’s services as an active participant in triage assessment and care coordination via the phone and in person for mental health services and cross-coverage for medical services. This position reports to the RN Nurse Manager.
ESSENTIAL FUNCTIONS: 1. Clinical Nursing Functions: 85% • Perform telephone and in-person assessments of people with acute psychiatric concerns to determine a safe, timely and appropriate disposition using evidence based best practice guidelines, decision support tools, nursing protocols, and psychosocial interventions. Triage will include both psychiatry and medical health patients. • Complete medication refills per protocol. • Respond to in-clinic emergency situations and urgent patient needs. • Administer care and treatment to children and adults with mental illness per provider care plans with close collaboration with providers. • Provide follow-ups for abnormal labs and emergency room and hospital discharges. • Serve as the access point to the clinic, assisting patients in receiving care and determining needs for interdisciplinary care. • Be familiar with CUHCC’s services in order to accurately refer patients within CUHCC and to appropriate outside services. • Assure quality of care by managing patient panels, such as for patients on clozaril. • Provide patient education. • Prepare patient forms for signing by provider; examples include applications for basic living needs, housing and transportation, access to health care, disability, and other benefits. • Delegate tasks to other members of the care team as appropriate. • Obtain important clinical collateral information to support provider clinical decision making and coordinate care with group homes, schools, or other agencies that service our patients. • Provide short-term care coordination for patients.
2. Other Duties: 15% • Attend and participate in appropriate meetings. • Participate in quality improvement efforts as outlined in the QI/QA plan and in grants that fund these nursing positions. • Participate in continuing education to maintain licensure and attend education sessions relevant to daily work. • Serve as a preceptor/mentor to nursing students. • Orient residents and health care students at CUHCC. • Other duties as assigned.

Willingness to work 4-8 hour shifts between the hours of 7 a.m. and 8:00 p.m. Monday through Friday and some Saturdays from 9:00 a.m. to 1:00 p.m. Will consider four 10 hour shifts for some positions. 9. Willingness to work with clients with medical and behavioral health needs, both in-person and on the phone. 10. Must be willing to work in a fast-paced, rapidly changing environment that requires sitting, standing, and walking, with frequently changing priorities. Must have a flexible mindset and have strong multitasking skills. 11. High-level communications skills to work with patients, staff, providers, and outside agencies. 12. Excellent customer service skills. 13. Willingness to work with patients and staff from diverse cultures, languages, and backgrounds. 14.
