The Caseworker provides a comprehensive needs assessment and service coordination to assist older persons in gaining access to, and receive, needed services.
Essential Duties & Responsibilities
Provide initial professional evaluation to determine client needs and provide guidance and/or referral to appropriate services. The setting will in a private home.
Develops service/treatment plans for clients
Arranges and coordinates services
Collaborate with other staff to discuss clients’ best interest
Manages a caseload, including monitoring of related service delivery
Provides ongoing support to clients and families as needed
Maintain complete, accurate, and timely casework documentation
Participates in related special projects and other duties as assigned.
Bachelor Degree from an accredited institution in social work, counseling or related field
Sound computer skills including Microsoft Word, Excel, Outlook, etc.
Excellent written, verbal and interpersonal communication skills with ability to communicate effectively with a diverse population
Excellent project management skills, including ability to manage multiple assignments with ability to prioritize and meet deadlines
This position requires a car and proof of auto insurance.
