Reporting to the Associate Director Population Health, the Project Manager for the Population Health Program will provide administrative, project management and data analysis support for achieving success in quality programs for the ColumbiaDoctors' commercial patient population. The Project Manager will utilize a data driven approach to coordinate all deliverables set forth by existing commercial payer contractual agreements related to quality and efficiency requirements. He/she will work collaboratively with the population health team including analysts, population health coordinators, care managers, providers and administrators in various departments, monitor key metrics across incentive programs and support all quality initiatives relevant to ColumbiaDoctors' patient population.
Work collaboratively with ColumbiaDoctors Quality Division, New York Quality Care ACO, Weill Cornell and New York Presbyterian Divisions.
Administrative responsibility for managing the day-to-day maintenance of commercial payer quality, claims and performance data including transfer of claims data, reports, etc... via SFTP and/or designated portals.
Apply project management skills to ensure successful achievement of all deliverables related to commercial payer incentive programs.
Adhere to established conventions related to saving files, naming and location of shared files.
Monitor, review, track, and analyze key performance metrics for all relevant programs available to ColumbiaDoctors' attributed patients.
Prepare and distribute reports on incentive program performance to participating practices and/or providers as directed.
Track incoming incentive payments including PMPM care coordination payments, quality incentive payments and shared savings and calculate distribution of same.
Actively participate in assigned population health/care coordination strategy meetings.
Collaborate with quality analysts to support data needs and analytics related to achieving success in commercial payer programs.
Maintain knowledge of current measure specifications and contribute to dashboard designs.
Participate in site visits, perform chart reviews, outreach activities and other duties as assigned.
Conforms to all applicable HIPAA, Billing compliance and safety guidelines.
Requires bachelor's degree or equivalent in education and experience, plus four years of related experience.
2-3 years of experience in the ambulatory health care industry, commercial payer industry and/or population health care.
Strong computer skills and proficiency in Microsoft Office Suite, with exceptional knowledge in Excel, familiarity with other analytic/statistical tools and systems a plus - such as SAS, SPSS, Tableau, Power BI, etc.
Experienced user of electronic medical records (Allscripts Touchworks and Epic preferred).
Comprehensive understanding of quality metrics such as HEDIS, PQRS, and NQF measures a plus.
Flexibility, tolerance and demonstrated ability to take initiative and to prioritize multiple tasks with attention to details.
Function at a fast pace and able to interact with a wide variety of people and situations while maintaining a customer focus.
Aware of scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources.
Excellent organization and communication skills with customer service delivery in a manner that patients are willing to accept.
Master's degree preferred.
Project management experience and analytics certification strongly desired
