Our Patient Discharge Navigator RNs demonstrate knowledge and skill in the facilitation of discharge of acute inpatients to the appropriate level of care with the proper level of support needed to avoid re-hospitalization.

As a Patient Discharge Navigator on the Ortho Medical Trauma Unit you know that every day is different, which is why you need to know how to:
Coordinate efficient and effective care between units and among interdisciplinary teams to ensure best use of resources and avoid duplication or fragmentation of services
Identify resources and roadblocks to insure efficient patient flow, best care and timely discharge
Collaborate with Case Managers to ensure proper discharge placement and support
Drive multidisciplinary and discharge rounds twice daily
Communicate professionally with patients, families, peers and providers
Promote mission, vision and values of SCL Health
Follow hospital, regulatory and professional guidelines, policies and procedures
We hire people, not resumes. But we also expect excellence, which is why we require:
ASN/ADN or BSN from an accredited program
Ability to obtain BSN within 4 years of hire unless you have a minimum of 15 years of proven, continuous acute care RN experience, required
Current RN license to practice nursing in the state of Montana
Three years of acute nursing experience, required
One year of Case Management or Discharge Navigator practice, preferred
