The FQHC (Federally Qualified Health Center) Outreach Project Manager will report to the Director, Community Hospital Partnerships & Affiliations. Under the divisional direction of Network and Affiliates and in partnership with respective academic departments and the office of Care Services and Innovation, this position will implement and operationalize a dynamic and efficient FQHC Outreach program in partnership with internal and external constituents, and future affiliated programs with community providers in accordance with the applicable regulatory requirements and business objectives of the organization. Incumbent will work with multiple stakeholders to assure the development of a highly reliable care delivery system.Required:-In-depth knowledge of population health, delivery system reform, and accountable care standards-Experience implementing and/or evaluating clinical operations and population-based health programs with knowledge of healthcare delivery in the field of FQHC, as well as principles, concepts, best practices, policies and processes, central to population health initiatives. -Advanced organizational and project management skills, with the ability to lead a team, prioritize tasks, and direct projects from inception to completion-Advanced critical thinking and problem-solving to evaluate highly-complex information, organizational matrix, assess problems, and develop effective short-range and long-range solutions-Strong knowledge of Microsoft Office and other specialized applications such as Adobe Acrobat and Visio-Strong skills in written, verbal, and interpersonal communications with individuals and groups of all levels throughout the organization.Preferred: -Advanced degree(s), such as MBA, MHA, MS, or similar -Work experience in Federally Qualified Health Centers or like-organizations-Community outreach experience-Academic Healthcare and community integration experience SPECIAL REQUIREMENT -Must be able to work flexible schedule as needed.-THIS POSITION MAY BE SUBJECT TO A CRIMINAL BACKGROUND INVESTIGATION, DRUG SCREEN, LIVE SCAN FINGERPRINTING, MEDICAL EVALUATION CLEARANCE, AND FUNCTIONAL CAPACITY ASSESSMENT
