NYC Health + Hospitals/Gouverneur provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly and infirmed in its 295-bed Skilled Nursing Facility (SNF). NYC Health + Hospitals/Gotham, Gouverneur’s D&TC is the largest freestanding D&TC in New York, providing more than 250,000 visits per year on the Lower East Side. Gotham, Gouverneur provides a comprehensive array of on-site services, including primary care, pediatrics, women’s health, dentistry, behavioral health, podiatry, optometry, ENT, digital imaging, infectious disease, and other specialties and sub-specialties. NYC Health + Hospitals/Gouverneur Skilled Nursing Facility provides 24-hour care for individuals in need of short-term, long-term care or hospice care. The facility is rated as a Best Nursing Home by US News and World Report. Post-Acute Care services include physical therapy, occupational therapy, speech and audiology, and vocational rehabilitation.
Under the direction of the Director of Admissions, the Senior Admissions Nurse Liasion provides facilitates admission activity for 5 post-acute facilities. Directly responsible for admissions for assigned facility(s) within the framework of established policies and procedures governed by the New York State Department of Health. Provides direction and leadership to department in absence of director. Provides clinical guidance to director and admitting staff in relation to patient/resident admissions, transfers and discharges throughout the Post-Acute Care Service Line.
Facilitates the admissions activity for assigned facility in accordance with resident needs, government regulations and established policies so as to maintain quality care for the residents while achieving the facility's business objectives.
Serves as the primary contact to receive all necessary referral information to determine patients' potential of medical necessity for admission into the long-term acute hospital or nursing facility
Enter patients/residents admitting and discharge diagnosis information timely when provided by Health Information Management coder. Validate entry through covered/non-covered MDS forms, e-mails and closed medical records.
Utilize the scan system to distribute patient/resident admitting information on day of arrival, ACB form for projected admissions and filing of admitted and not admitted PRI's.
Ensures compliance with needed referral information from referral sources in order to make a timely decision regarding admission and have at-hand for in-house physician inquiry.
Ensures that appropriate processes are in place to monitor and maintain systematic, timely credentialing and re-credentialing of medical providers. Provides support to credentialing committee as required.
Participates in preparation for regulatory agency audits and surveys; responds to information inquiries as required.
Assists with developing and promoting for marketing activities for including building referral source relationships to increase referral volumes.
Assists with monitoring overall efficiencies of the Centralized Admissions Department including responding to referrals faster and more thoroughly.
Performs related duties as needed.
1. Licensed and currently registered to practice as a professional nurse in New York State, plus (2) or (3), below.
2. Masters Degree in Nursing Administration, Education, Clinical Specialty or equivalent field and five years of satisfactory nursing experience, of which three years shall have been in a supervisory or teaching capacity; or a Baccalaureate Degree in Nursing and six years of satisfactory nursing experience, four years of which shall have been in a supervisory or teaching capacity.
3. A satisfactory equivalent of education and experience. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
Previous experience in an Admissions capacity
Experience working in nursing capacity in long-term care setting
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
Must be able to work in a fast-paced environment and take appropriate action
Strong communication skills, both oral and written.
Ability to work in a collaborative, team oriented environment.
Proficiency in MS Office Suite (Outlook, Word, Excel and PowerPoint)
