Access to Care is a non-profit organization that provides primary healthcare services to low-income and uninsured residents of suburban Cook County and northwest Chicago. We have been serving the community for over 30 years and are looking for an experienced, out-going, full-time Outreach Coordinator. The primary responsibility is to increase awareness of the Access to Care program throughout our service area. This position must be bilingual (English/Spanish), have excellent verbal communication skills necessary to communicate complex concepts in a clear, concise manner with all levels of staff, patient population and the community composed of diverse cultures and age groups.
Primarily responsible for reaching the underserved population within the Access to Care service area. Create and execute a strategy for reaching and serving diverse communities; with the ultimate goal of increasing Access to Care enrollment.
Assist with educating community partners about Access to Care and its importance to their constituents and the community. Increase awareness and promote the program.
Responsible for generating, developing, and cultivating relationships in faith-based communities, community members, schools, and health and human service agencies.
Educate and network with people of diverse backgrounds and cultures.
Develop and implement targeted outreach activities, attend events, and generate speaking engagements which will attract potential Access to Care members.
Assist Public Relations Manager with special projects and new initiatives.
Ability to transport and set up display and promotional materials.
Participate in social media forums to extend the program footprint.
Ability to work independently with minimal supervision.
Bachelor’s Degree in a health or social services/social sciences or related field is preferred, or equivalent occupational experience will be considered.
Three to five years of outreach experience, a healthcare background; non-profit organization, governmental entity or the private sector with an emphasis on working with diverse ethnic and socio-economic status communities.
Bilingual and literate (English/Spanish), required.
Excellent organizational skills in independent managing workload and related responsibilities.
Experience delivering presentations, comfortable with public speaking.
Ability to travel to events throughout Cook County, work flexible hours including evenings and weekends.
Social Media management.
Strong computer skills including proficiency with Microsoft Office Suite.

The Suburban Primary Health Care Council has, through its Access to Care program, been filling the gap for the uninsured and under-insured individuals of Cook County for 30 years. Serving more than 5000 low income members every year, Access is to Care is a primary care safety net for those individuals not eligible for public assistance. Increasingly we are also serving the under-insured, those who cannot afford the high deductibles of their insurance plans.
