Join the AMA team as an Improvement Specialist in Improving Health Outcomes. As part of the team, you will provide technical expertise and training for the implementation of clinical programs in health care organizations for the Improving Health Outcomes (IHO) Clinical Service Team. Helps achieve IHO and Clinical Service Team goals by conducting data-driven quality improvement (QI) coaching, ongoing training, consultation and monitoring of primary care practice sites participating in IHO QI interventions. Works with health systems, primary care practices, health centers, and community partners to achieve clinical and operational improvements in cardiovascular disease prevention strategies (improving blood pressure control, preventing type 2 diabetes, and treating dyslipidemia).
Other Responsibilities will include:
Provide training of clinical trainers for healthcare organizations (HCOs) participating in IHO Clinical QI Programs - Serve as a core member of the implementation team for national IHO Clinical QI Programs and Initiatives, lead training sessions to train HCO designated trainers including the creation of training materials, agendas and supportive materials. Oversee the management and coordination of training events. Facilitate learning using multiple modalities (webinars, in-person presentations, workshops, written newsletters, reports, etc.) to transfer knowledge of best practices and overcoming common implementation challenges facing trainees. Conduct formal, professional presentations, trainings and workshops on quality improvement methods to support the implementation of the AMA’s M.A.P. (Measure, Act, Partner) interventions. Provide guidance and direction representing the interests of clinical practice sites in measurement and evaluation activities.
Coach Individual Clinical Practice Sites - Help provide customized coaching to clinical practice sites by identifying site-specific needs and opportunities, prioritizing areas of focus, and identifying available and needed resources. Address barriers to effective implementation of improvement processes; propose solutions for practices and community health centers to consider in addressing common barriers. Advise individual clinical practice sites on the implementation of IHO MAP interventions and evaluation measures, sharing specific best practice examples. Work with clinical practice leadership to identify existing data sources to minimize the burden of data collection and identify opportunities to efficiently retrieve data to support QI activities.
Contribute to the Development of IHO’s Spread Strategy - Interact with community partners, such as retail pharmacies, clinics and others, to develop broad, sustainable strategies for improved health outcomes. Develop innovative ideas and training tools for future spread of best practices. Contribute to the development and implementation of IHO’s strategic approach to spread its clinical QI programs locally, regionally and nationally.
Collaboration and Teamwork - Communicate the experiences of the individuals trained and the practices they serve to the larger Clinical Service Team, Operations Team, and Client Service Team in addition to the overall IHO leadership team to ensure that decisions impacting HCOs and practice sites are in their best interest, aligned with their internal goals, and are able to be supported through resources made available to them. Ability to work with all IHO leadership, Advisory Groups and others as needed to further refine IHO interventions, evaluations and associated tools for use in IHO QI Program spread efforts. Work across the organization to support IHO strategic goals for cardiovascular disease prevention (addressing abnormal blood pressure, glucose, and dyslipidemia) on matters of practice sites’ needs and experiences and satisfaction, including contributing to planning, budgeting, staffing, and other core organizational decisions. Effectively lead all projects assigned by gathering requirements, developing solutions and providing collaborative guidance and support to various other IHO teams.
Master’s degree in nursing, public health, health care administration, social work or health-related field preferred or equivalent, applicable work experience.
Minimum 5 years’ experience in the health care field preferably in primary care practice, quality improvement or public health.
Minimum of 3 years of experience in the planning and leading quality improvement initiatives and, coaching practices in change concepts and measurement strategies which led to demonstrated achievements in improving health outcomes; strong emphasis on experience related to effectiveness, sustainability of improvements and spread of initiative.
Demonstrated expertise in quality improvement principles and practice, preferably in an ambulatory clinical practice setting; exposure to organized medicine, regulatory issues, and related organizations preferred.
Experience with Clinical informatics and using electronic health records (EHR) and EHR report writing preferred.
Ability to communicate effectively with primary healthcare teams including leadership and success in influencing changes in processes and practices; ability to recognize the relationship between primary healthcare and the complex systems of healthcare delivery.
Effective project management skills; with the ability to effectively manage time; prioritize work, multi-task across many assignments and manage expenses within a budget; ability to handle ambiguity and adapt effectively to changing circumstances.
Strong analytical and critical thinking skills required.
Excellent interpersonal skills, strong written and verbal communication skills, including experience presenting in a large group setting.
Some travel required; travel may be as high as 30%.
