The Integrated Care Program at Boston Children's Hospital works to develop and implement tools, measures, and processes to improve care coordination and integration for patients and their families. Our team is seeking a project manager who will support the implementation such activities with the subspecialty departments at Boston Children's Hospital and in collaboration with our community based referring partners.
